Your First Visit – a note from Dr. Dolgos

Your first impression is very important to me. I promise that you will have my undivided attention as you describe the history of your present condition. You will have time to discuss any concerns or fears you have, as well as your hopes and objectives. You will have the opportunity to review any relevant medical history. Only then, with your permission, will I perform a detailed head and neck examination. With this information, you and I can begin to understand what factors might have brought you to this point.

Frequently, additional diagnostic information, such as MRI’s, CT scans, and electrodiagnostic data, is recommended before an appropriate plan of action can be proposed.

Once the necessary information has been gathered, you and I will have a chance to review this information together, and your treatment options can then be discussed.

Please assist me by providing the following information at the time of your consultation:

IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian.

Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.).

X-Rays

If your dentist or physician has taken x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional x-rays are necessary, they can be taken at our facility.
